You've heard about Wegovy, Mounjaro, and the class of medications people call "weight loss injections." What you actually want to know: does this work for my situation? What does supervised treatment look like? What happens when you stop? This page gives you honest answers.
I've seen many patients who sourced these medications on their own and self-injected. That's not something I support, not because the medications don't work, but because using them without monitoring isn't really using them properly. I'm willing to work with you on GLP-1 therapy because I think that's what actually helps.
GLP-1 (Glucagon-Like Peptide-1) is a hormone your small intestine naturally produces after eating. It signals the brain that you've had enough, stimulates insulin release, suppresses glucagon, and slows gastric emptying. In people living with obesity, this signaling system is often dysregulated, the GLP-1 response is weaker or the brain's receptors are less responsive. The result: satiety arrives late, and appetite stays elevated.
GLP-1 receptor agonist medications work by reinforcing this signal from the outside. Modern formulations have a much longer half-life than natural GLP-1, weekly injections maintain therapeutic levels continuously, without the spikes and drops of the body's own secretion.
The newest generation of medications (dual incretins like Tirzepatide) act on both GLP-1 and GIP (Glucose-dependent Insulinotropic Polypeptide) receptors simultaneously. This dual mechanism produces more pronounced weight loss and metabolic improvement than GLP-1 alone in head-to-head trials.
"These medications don't work by making you too nauseous to eat. They work by reducing the desire to eat in the first place. That's a meaningful distinction."
The reason I recommend GLP-1 therapy when it's appropriate is that it acts at the right level. It's not a substitute for willpower. It's a tool that makes willpower less necessary. There's a significant difference.

The FDA approved oral Wegovy (Semaglutide, once-daily fasting) in December 2025, and Foundayo (Orforglipron, once-daily, no fasting required) in April 2026. Both are now available in the US. Taiwan regulatory approval is pending.
Taiwan's existing Rybelsus is an oral Semaglutide, but it's approved for type 2 diabetes and carries strict fasting requirements, it's a different positioning from these next-generation weight-management oral agents. Oral formulations are expected to significantly expand access for patients who prefer not to inject.
Not everyone needs it, and not everyone is a good candidate. Here's how to think about it before coming in.

These don't automatically rule out GLP-1 therapy, they require disclosure and individual assessment:
GLP-1 therapy isn't simply "available if your BMI qualifies." Before prescribing, I look at your body composition, metabolic markers, medication history, and what your daily life actually looks like. That's not gatekeeping, it's making sure the choice actually makes sense for you.
Real clinical answers, not generic information pamphlets.
GLP-1 medications are prescription-only in Taiwan and most other countries. Obtaining them without a prescription (whether through gray-market online sources, overseas mail order, or informal group purchasing) carries real risks beyond the legal question.
Cold chain integrity. These injectable medications require continuous refrigeration between 2–8°C. There's no way to verify that supply chain conditions were maintained during an unregulated purchase. Degraded medication won't just be less effective, you won't know it's degraded.
Dose titration requires medical oversight. GLP-1 therapy starts at a low dose and is incrementally increased every 4–8 weeks based on your individual response. Without someone managing that process, side effects are harder to handle and the risk of stopping prematurely (or escalating too quickly) goes up significantly.
It's not a standalone intervention. In my practice, GLP-1 therapy is combined with dietary adjustment, regular body composition monitoring, and follow-up. Injecting without any of that framework reduces the likelihood of lasting results and increases the chance of weight returning after stopping.
You have the freedom to make your own choices. If you're going to use these medications, I'd prefer you do it with proper medical support.
Most people notice meaningful appetite change by weeks 4–12. With good dietary adherence, the first three months typically bring 8–10% body weight reduction. Over a full treatment course: GLP-1 single agents (like Wegovy) average around 15% in major trials; dual incretins (Mounjaro/Tirzepatide) have reached over 20% average in large-scale studies.
Weight regain after stopping is real, and it's not a treatment failure. GLP-1 medications change the brain's satiety signaling while you're on them, when you stop, that signal gradually returns to its previous baseline. This is why building actual lifestyle habits during treatment matters so much. Some patients stay on a low maintenance dose long term; others taper off and continue monitoring. We work out the right strategy at your follow-ups.
The most common side effects are gastrointestinal, nausea, vomiting, bloating, constipation. These are most pronounced when starting or increasing the dose, and most patients adapt within 2–4 weeks. Eating smaller portions and avoiding high-fat meals helps significantly. We go through this in detail before you start.
Contact the care team promptly (don't wait for your next appointment) if you experience: severe persistent abdominal pain radiating to the back; jaundice or darkening of urine; rapid heartbeat with neck or jaw pain; severe allergic reaction; or sudden vision changes (especially in diabetic patients).
On long-term safety: the SELECT trial (17,000+ participants) demonstrated cardiovascular protective effects of Semaglutide in high-risk patients, with no serious long-term harms identified. Pancreatitis risk is a monitoring point for patients with relevant history, which is why pre-treatment assessment matters.
These aren't competitors, they're different tools at different stages of the same spectrum.
If your BMI is in the 27–32 range without severe comorbidities, or if you're not yet considering surgery, GLP-1 therapy is a strong primary option. It's reversible, non-invasive, and has a substantial evidence base.
If your BMI is above 32.5 with significant comorbidities (type 2 diabetes, hypertension, sleep apnea) the clinical data for metabolic surgery is more durable in terms of long-term weight maintenance and comorbidity resolution. Surgery doesn't require ongoing medication, and its effects on diabetes remission are particularly strong within the first five years.
A third path is using GLP-1 therapy first to reduce liver fat and body weight before surgery, lowering operative risk. That's a completely legitimate strategy.
The honest answer: the right choice depends on your specific situation. That's not a deflection, it's the reason a consultation exists.
GLP-1 therapy for weight management is paid out of pocket. If you carry private health insurance, whether it reimburses this kind of treatment varies by policy, check with your insurer before starting.
The cost varies by agent, dose, and duration. At current market rates, expect several thousand NTD per week of treatment. During your consultation, I'll outline the expected duration and total cost based on your situation so you can make a fully informed decision.
A word on shared pens, this is a genuine safety issue.
There have been cases of people splitting the cost of a single pen between multiple users. Even with separate needles, there is a real risk of blood backflow contaminating the medication cartridge, and subsequently transmitting bloodborne infections including Hepatitis B, Hepatitis C, and HIV to the next person who injects from the same pen. This has caused documented medical infections, not just theoretical risk.
GLP-1 injector pens are single-patient medical devices. They cannot be shared. Cost concerns are something we can discuss, but not by trading infection risk.
GLP-1 therapy in my practice is already combined with several things: dietitian-led dietary adjustment, appropriate exercise, regular InBody body composition tracking, and concurrent management of any metabolic conditions.
If you're on other medications affecting blood glucose or weight, these need to be disclosed before starting, drug interactions matter. Unregulated weight-loss supplements or products with unknown active ingredients are not recommended alongside GLP-1 therapy.

Taiwan already has one oral GLP-1. Rybelsus (oral Semaglutide), approved in 2022 for type 2 diabetes. It requires strict daily fasting conditions (30-minute window before eating or other medications) and is not approved for weight management specifically.

Don't write it off yet. Body weight is affected by fluid shifts, digestive state, menstrual cycle, sleep, and sodium intake, the scale number today is not the full picture.
Three signals more reliable than the scale: Has your waist measurement changed? Has your appetite shifted, are you stopping at 70% full more often? Has the frequency of overeating or late-night eating decreased? If two of these three are trending better, you're usually not failing, you're in the early foundation phase.
One common issue: many patients conclude "it's not working" while still in the dose titration phase. GLP-1 medications are designed to start low and build gradually, you're not at full effect until you've reached the target dose. Weight change during the ramp-up period is inherently slower.
The other factor: regular follow-up matters. A proper clinical review (body composition, dose assessment, dietary analysis) gives a much more accurate picture than self-monitoring alone. Come in before drawing conclusions.
Plateaus are a normal metabolic response, not a signal that the medication has failed. As weight drops, the body downregulates its basal metabolic rate as a protective mechanism. A weight plateau doesn't mean GLP-1 has stopped working, it means the body is resetting its equilibrium point.
The two most common mistakes at this stage: stopping the medication on your own, or insisting on a switch to a stronger agent. Both often waste the progress already made.
The right move is to come in. A proper assessment looks at: how body composition is shifting (is fat still decreasing even if weight isn't?), whether dietary adherence needs adjustment, whether dose has room to increase, and whether adding another strategy makes sense. This is also when dietitian input is most valuable, metabolic adaptation is partly a dietary strategy problem.
You can pursue GLP-1 therapy at BMI 32+, but you need a realistic picture of what to expect. Clinical experience shows that the relative weight reduction from GLP-1 agents (typically 10–20%) may not bring someone with a high starting BMI into a genuinely safe metabolic range. And the regain rate after stopping tends to be faster and more pronounced at higher starting weights if lifestyle changes haven't been solidified.
This doesn't mean GLP-1 therapy has no role, it means that at this BMI range, metabolic surgery deserves serious consideration as part of the discussion, not just a distant option. Two frameworks that make sense:
GLP-1 first, then reassess surgery: Use medication to reduce liver fat and body weight first, lowering surgical risk. Then evaluate whether surgery is still needed or appropriate. This is a completely valid pathway.
Go directly to surgical evaluation: If your BMI is 32.5+ with comorbidities (diabetes, hypertension, sleep apnea), the long-term data for metabolic surgery on weight maintenance and comorbidity resolution is meaningfully stronger than medication alone.
This is a decision that benefits from a proper consultation, body composition, comorbidity profile, and lifestyle context all factor in. Come in and let's look at the numbers together.
GLP-1 medications have side effects. Most are mild to moderate gastrointestinal symptoms and are temporary. Knowing what to expect is better than being caught off guard.

If you're using any GLP-1 medication, always tell the anesthesiologist before any procedure requiring anesthesia, including gastroscopy, colonoscopy, or any surgery. Provide the medication name, dose, and the date of your last injection.
GLP-1 medications slow gastric emptying. Even after an 8-hour fast, there may still be residual contents in the stomach, raising the risk of aspiration under anesthesia.
As GLP-1 reduces your food intake, your basal metabolic rate may also decrease, the body's adaptive response to lower caloric supply, which involves losing both fat and muscle mass.
This is why dietitian involvement and body composition monitoring are non-negotiable in my program. Adequate protein intake, regular InBody tracking, and adjusting the dietary strategy before stopping medication are all part of protecting your metabolic baseline.

What I'm tracking isn't just how many kilograms you've lost. It's where your body fat is, how much muscle you've retained, and whether your metabolic markers have genuinely improved. Those are the numbers that tell me whether treatment is working.
GLP-1 therapy isn't the beginning or the end. It's one level in a tiered approach, more intensive than lifestyle changes alone, more conservative than surgery.
Some patients reach their target weight with GLP-1 therapy and maintain it from there. Some use it as a bridge before surgery, reducing liver fat and operative risk first. Some, after a course of therapy, decide they want a more durable solution and move to a surgical discussion. All of these are valid paths. None of them represents failure.
GLP-1 therapy can also be used after bariatric surgery, when weight regain exceeds 10%, incretin medications are among the most evidence-supported adjunct options available.
